1. Requesting Cancellation
- Notification Requirement: Please submit a written cancellation request at least 30 days prior to your desired cancellation date. You may email us directly or use our official cancellation form if one is provided.
- Consultation Meeting: We request a brief exit meeting to discuss any challenges you encountered. This helps us improve our programs and better understand your needs.
2. Cancellation Fees
- Cancellation Fee: If you choose to cancel before completing the full monthy commitment, an early cancellation fee of 25% of the remaining contract balance will apply. This fee covers resources and planning dedicated to your wellness journey.
- Waiver for Extreme Circumstances: We understand that certain circumstances, such as major health changes, or loss of employment, may impact your ability to continue. In such cases, we may consider waiving or reducing the cancellation fee.
3. Settlement of Outstanding Payments
- Final Payment Due: Any outstanding balances, including services rendered and the cancellation fee, must be settled within 14 days of the cancellation request being approved.
- Refund Eligibility: Clients who have prepaid for services beyond the cancellation date may be eligible for a prorated refund, minus the cancellation fee.
4. Termination of Services
- Services will be terminated on the agreed-upon cancellation date.
- Access to resources, programs, and client portals will end upon cancellation.
5. Re-Enrollment Options
- Should your circumstances change, you are always welcome to re-enroll. We’ll be happy to discuss flexible options to help you get back on track.
